Filling in this form  in accordance with the requirements of Article 9 of Federal Law No. 152-FZ of July 27, 2006  Personal Data", I confirm my consent to process  the personal data entered into the form by the person rendering services on the basis of this form (hereinafter referred to as the Operator)

2) contact phone number;

3) e-mail address.

I give the Site Administrator the right to carry out all actions (operations) with my personal data, including collection, systematization, accumulation, storage, updating, modification, use, depersonalization, blocking, destruction.

The purpose of processing personal data is to provide services to me based on the completed form.

The operator has the right to exchange (receive and transmit) my personal data using computer media or through communication channels, with the observance of measures to ensure their protection against unauthorized access.

This consent is valid for an unlimited period, the period of storage of my personal data is not limited.

I reserve the right to withdraw my consent by drawing up an appropriate document, which can be sent to me by e-mail: remmontag-k-ur@mail.ru to the site administrator.

In the event of withdrawal of consent to the processing of their personal data, the Site Administrator undertakes to delete my personal data within a period not later than 3 working days.
